The intervention is a streamlined survey and reporting tool designed to improve communication between cardiovascular patients and physicians. It collects and summarizes relevant health factors, promoting personalized, patient-centered care that considers a fuller picture of the patient’s well-being.

IMPROVE CV Care

HOW IT WORKS

Before appointments, patients complete a brief survey capturing mental health, social support, and self-care using validated tools. The results generate a report that flags key changes and provides insights for clinicians, helping them quickly understand the patient’s overall health and support needs beyond immediate symptoms.
